If it is always the same false axle box it could be that the clios that hold it in place are a bit on the weak side, we had one that was always loosing the same part in the Ruschbahn but not after a derailment just anywhere on the line. I would think that the faux axlegaurd might be fouling the axle's pivot mechanism as it detaches. Anything that interferes with the guided movement of that pivoted axle, by its attached coupler arm, could lead to a derailments. I remember a Piko Scheinenbus pair that I had, each car had a similar pivoted, both motorised and unpowered, pair of single axle "bogie" set up. It was delivered without the correct flanged pickup skates, used as the axle "bogies" were not attached to couplers, to keep the wheels tracking straight. Just would not work without them to maintain accurate tracking, they just flailed around. Waddya want - re-sale value or a wagon that works?I have never been in a position to observe an actual derailment, it's either in the far distance, or the wrong side.
There is no pattern to it, points, curves and straights, it runs for days OK, but then sometimes, it carn't make a single lap, without falling off.
As for glueing the offending piece back in, you would of thought if it drops out it should just push back in, without having to strip the body off.
I'll be more intrested as to seeing the mechanics of how it dislodges itself, so I can reverse the process, it must twist somehow to come out.
As it's a limited edition item I am loathed to modify it, after all it will be disposed of some day, but then again, I probably won't be around, so maybe the glue will be pressed into action, as I will not be in a position to care.

Try a small piece of Blue Tac, or similarAs for glueing the offending piece back in, you would of thought if it drops out it should just push back in, without having to strip the body off.
