I have the loco on a piece of track on a shelf at eye level at the moment. I noticed that the flangeless middle driving wheels are slightly clear of the rail head. (By maybe 1/10 of a mm- just see daylight) Normal? Performs very well and I suspect in use all 6 are rarely all down it not being sprung anyway.

That's normal. Prevents rocking on uneven track.

Yes - quite normal. Same system used on old Tri-ang 00 gauge.
I've recently removed flanges from the centre wheels of my Accu Wrekin & Lawley, and took a little of the tread - possibly not enough as there is a little hint of rock at the peak of a gradient.
